I purchased your K2 camera system on February 19, 2019, and the system worked flawlessly until last week, when I noticed that the rear camera has stopped working. I have attempted the following steps to troubleshoot:
1) Updated to the latest firmware. I tried both “k2.20190614.Vo.57” and “k2.2019.08.23.Vo.61”, and got the same result both times.
2) I swapped the camera connections on the DVR, moving the front camera wire to the rear slot, and the rear camera wire to the front slot, to see if the fault would follow, meaning to see if the rear camera would record as if it was in the front.
This is where it gets interesting. After I swapped the connections, the DVR would get stuck in a boot loop. After powering on, the camera light and video light would alternately flash red and green, the GPS light would not come on at all, and the WiFi light would flash on and off, and then the DVR would power off and back on.
After powering the motorcycle off, and switching the camera wires back to the original configuration (front camera in front slot, rear camera in rear slot), the DVR would power on normally, the GPS and WiFi lights would come on and stay on, and the green video light would stay on, and the front camera would begin recording.

Rear Camera not working. Looks like water ingress via the lense. Out of warranty so have to pay for replacement.
Seems to be a pattern.

This happens if there is no front cam connected to the DVR and normal so far.
Your rear cam stopped working. You then plugged it into the front cam slot to check whether the error follows or not. And it seems it did, as the behaviour mentioned above is normal if there is no camera connected.
You could check if there is something wrong with the plug of the rear cam. If not, you should contact Innovv for a replacement.
